Intake and Assessment Clinician Hutt South Community
Location: Public Trust House, Queens Drive Lower Hutt
Contract: Full Time, Permanent (1 FTE) Mon-Fri

About the role
The Intake and Assessment Clinician’s role helps support facilitating access to mental health services in partnership with the people we support within the community.
Key priorities of the role include:
- Triaging referrals from a variety of sources in the community
- Undertake face to face initial assessments if indiciated in the triage process
- Recognise that culturally safe practices are paramount, and liaise with relevant services to ensure people receive safe, high quality and equitable care
- Assess and escalate any crisis/urgent referrals to the appropriate crisis mental health and addiction services
- Works collaboratively alongside the community mental health teams and local service providers to access the most appropriate service/support for the person based on their requirements
- Liaise with referrers, including local GP’s and community networks/NGO providers in the region to ensure people get appropriate, timely and person centred-care and feed back to them if there are any issues regarding quality of referrals
About the Team/Service/ Location
We are an experienced mental health team situated on the Hutt Valley. We consist of Psychiatrists, psychologists, social workers, occupational therapist, alcohol/drug clinicians, registered mental health nurses, clinical nurse specialist and admin staff. We have visiting NGO’s, lived experience lead and other teams from Wellington.
We are a team who likes who takes pride in providing expert care and treatment to our clients and have an excellent working relationship with our local NGO’s and community services.
We have parking and public transport close by. We are a team that like to celebrate each other’s successes.
About you
We need:
- An experienced registered mental health professional (allied health or nursing) with recent acute experience in mental health.
- An up-to-date annual practicing certificate (must have completed relevant post grad qualifications related to mental health).
- 2-5 years of experience in mental health triage and risk assessment in your relevant scope
