Clinical Coordinator – Community Triage


Location:  Based at the Kenepuru Community Health Building

Contract:  Permanent, Part Time, 0.45 FTE

About the Role

We have an exciting opportunity for an experienced Allied Health clinician looking to step into a leadership role, to join our Community ORA Service as our Triage Clinical Coordinator.  The coordinator role is 0.45 FTE – with 8 hours release time for Coordinator work and the remaining time clinically triaging.  The Triage team are part of the community ORA service, receiving the referrals (both Allied and Nursing) to the wider service.  The team uses a person-centred approach, discussing with the person referred what their needs are and how they can best be met by our teams.  Speaking directly to the person referred allows us the opportunity to work collaboratively with people around their self-identified needs and gives us the opportunity to identify if we can provide early intervention from Triage to best meet their needs, and then identifying and moving the referral to the part of our service that best supports the patient.

The coordinator supports the clinical coordination of the service, matching staffing with referrals and support the development of the Triage competencies as well as tweaking and developing the processes and systems aligning with triage.  The coordinator supports the Triagers in the team to ensure staff development, consistency of practice and providing support.  Stakeholder management and building relationships with the community teams as well as referrers will be key. The coordinator also links in with other services across the hospital who we touch with (e.g. other outpatient departments, discharging wards etc) to ensure smooth transition of patient care and supporting navigation of referrals for patients. 

The role is based at Kenepuru Community Health Centre – but there is opportunity to be based at the Wellington and Kapiti team sites as well.

About the Team/Service/ Location

The ORA Community Services work with people aged 16 and above with acquired neurological, long term, or age-related conditions including stroke, dementia, frailty and post trauma conditions.  The teams are multidisciplinary including occupational therapy, physiotherapy, speech-language therapy, social work, dietetics, liaison nursing and allied health assistants, with close links with Nurse Practitioners, the AWHI (ESD) team, Geriatricians and the Rehab Physician.  There are three community teams geographically located in Wellington, Kenepuru and Kapiti.  The Triage service sits within these community teams as the one-point entry to the teams.

About you

This role would suit an Allied Health Clinician who has experience of working in a multi-disciplinary team.  You will be keen to further develop your clinical reasoning skills and ability to identify, and problem solve needs and risk.  It will suit someone who likes to look at people’s needs holistically and really listen to their concerns and is passionate about our ability to make a difference to people’s lives. 

Communication and organisational skills will be a strength of yours, as well as the ability to build relationships quickly and work well with the triagers, referrers, the community ORA teams, your colleagues in primary care, other providers, clients and their whanau.

You will need:

  • New Zealand Registered clinician with at least 5 years clinical experience and a current APC
  • A clear understanding of the health needs of our Māori and Pacific Island communities
  • A driver's licence
  • A can-do attitude and a passion for working alongside people
  • Working visa/NZ residence (as appropriate)
