Kaiawhina Manaaki - Advancing Wellness at Home Initiative
Location: Wellington Regional Hospital
Contract: Permanent, Part Time (0.8 FTE / 32 hours per week, Rostered and Rotating)

About the Role
This role has been developed to work alongside the Advancing Wellness at Home Initiative (AWHI) Team in Wellington Regional Hospital and Kenepuru Hospital to provide cultural support to Māori patients & their whanau who may benefit from the AWHI service. The role will initially be based out of Wellington Regional Hospital/Ewart Building but also support transition into the community and follow-up with patients as required after discharge from hospital. The role will include meeting with patients and whanau in the hospital setting, visiting them at their home/in their community as well as completing some basic rehabilitation tasks as required during these visits. Training will be provided as required. This role will also be supported by and develop close links with our Whānau Care Service.
About the Team/Service Location
AWHI is Te Whatu Ora - Capital & Coast & Hutt Valley’s Early Supported Discharge service who deliver rehabilitation in the community. The AWHI Team work with patients who have been admitted to hospital following an acute illness, fall/injury or stroke. You will be joining a friendly, fun and enthusiastic team from a mix of professional groups. The AWHI team is split over 3 sites; Kenepuru Hospital, Ewart building at Wellington Hospital and Hutt Hospital; supporting patients across the district. All team members have access to on-going professional development opportunities including in-house & external training and access to supervision.
About you
- A fluent speaker of Te Reo Māori would be preferred.
- Comfortable in tikanga Māori and situations where Te Reo Māori is being spoken.
- Good communication skills able to sit and listen to whānau.
- Ability to work without supervision, to be highly self-motivated and prioritise customer service in a positive manner.
- Recent experience within a healthcare setting dealing with clients / whanau.
- Will hold a full New Zealand Driver’s license.
- Will hold or be willing to work towards relevant Level 3 Qualification (or equivalent).
