Nurse Practitioner – Primary Care Liaison Service

Location: Multiple localities – Wellington, Porirua-Kapiti, Hutt Valley and Wairarapa

Contract: Permanent Full time (1 FTE), Monday-Friday 8:30AM-5PM.

About the Role

The primary purpose of the role is to work independently and collaboratively with the primary care practices across the specific localities and the locality Community Mental Health teams as an autonomous practitioner. The Mātanga TapuhiNurse Practitioner (NP) role is integral to providing an individual and population-based approach to meet mental healthcare needs. This community based role improves access to mental health care provision for patients across the primary and tertiary care settings. This requires a flexible approach, to provide consultation, direct care and treatment enabling episodes of care management, as well as support through clinical advice and education.  The advanced practice role is integral to the provision and ongoing development of high-quality tangata whaiora care - that promotes optimal experience and equitable outcomes for tangata whaiora and their whānau.

About the Team/Service/ Location

MHAIDS spans two districts - Wairarapa, Capital, Coast and Hutt Valley and includes local, regional, and national services. The local MHAID services are provided from multiple sites within the sub-region – greater Wellington, Hutt Valley and Wairarapa. Te Korowai Whāriki services include regional forensic and rehabilitation services covering the Central region while the intellectual disability services extend the length and breadth of the country from six bases located in Whangarei, Auckland, Cambridge, Wellington, Christchurch, and Dunedin. The core business of the Service is the provision of quality specialist services, integrated with primary and secondary providers in the communities we serve.

The Mental Health, Addiction and Intellectual Disability Service (MHAIDs) has created a Primary Care Liaison (PCL) Service to strengthen the interface between primary care and secondary specialist mental health care. The long-term direction of the PCL service is to improve mental health service delivery within primary care settings and reduce demand for secondary services. The PCL team supports communication between services, support early specialist mental health service intervention, and offer timely clinical advice to primary care clinicians.

About you

• Clinical Nursing Masters including or additional NP required practicum course/s (60 credits).
• Mātanga Tapuhi NP Annual Practising Certificate with Nursing Council of New Zealand (NCNZ).
• Have at least four years’ experience aligning with mental health care.
• Sign-off when applicable for technical skills/certification /credentials/endorsements e.g. radiology endorsement
• If an international trained NP not registered  as an NP with the NCNZ please include university transcript for your Masters courses with your application. 
• Comprehensive knowledge of NZ Health System understanding including equity issues, professional leadership and emerging issues for the nursing profession.
• Excellent communication, interpersonal and facilitation skills.
• Experience in service development and quality improvement strategies.

We are looking for a recognised advanced practice nurse that has a passion for ongoing learning and the ability to adapt to work effectively with different learning experiences. You will need:

• Registered Nurse (RN) and a current practising certificate with no restrictions from the NCNZ
• At least three - four years relevant expertise clinical experience in mental health nursing and working at an advanced RN practice level aligning with mental health area of practice.
• Expert or Senior level on the Professional Development and Recognition Programme (PDRP)   
• A Clinical Masters or the prerequisites postgraduate courses (120 Credits) to meet the requirements  to undertake the NP postgraduate practicum course/s enabling application on to the national Nurse Practitioner Training Support Scheme (NPTSS).
