About the Role

The role of the Primary Care Liaison Nurse is to support primary care through increasing access to effective, expert advice for the clinical needs of clients experiencing moderate mental health issues within primary care. This is an autonomous role that provides direct advice, joint assessments, education and participation in care planning with a view to develop increased primary care capability. The nurse also has an active presence within the community mental health teams.

The primary care Liaison Nurse works with clients, mental health clinicians, GPs, practice nurses and other stakeholders to ensure that clients are able to access the treatment most appropriate to their needs. The PCL Nurse does not provide a care management role to clients, rather acts in a consultation role for primary care via GPs, Health Centres and CMHTs, as well as providing assessment and brief intervention when required. The primary care liaison nurse also has an active role in service development.

About the Team/Service/ Location

MHAIDS spans two districts - Wairarapa, Capital, Coast and Hutt Valley and includes local, regional, and national services. The local MHAID services are provided from multiple sites within the sub-region – greater Wellington, Hutt Valley and Wairarapa. Te Korowai Whāriki services include regional forensic and rehabilitation services covering the Central region while the intellectual disability services extend the length and breadth of the country from six bases located in Whangarei, Auckland, Cambridge, Wellington, Christchurch, and Dunedin. The core business of the Service is the provision of quality specialist services, integrated with primary and secondary providers in the communities we serve.

The Mental Health, Addiction and Intellectual Disability Service (MHAIDs) has created a Primary Care Liaison (PCL) Service to strengthen the interface between primary care and secondary specialist mental health care. The long-term direction of the PCL service is to improve mental health service delivery within primary care settings and reduce demand for secondary services. The PCL team supports communication between services, support early specialist mental health service intervention, and offer timely clinical advice to primary care clinicians.

About you

Essential Professional Qualifications / Accreditations / Registrations skills and experience:

▪ Registration with Nursing Council of New Zealand (NCNZ)

▪ A current Annual Practicing Certificate (APC) and scope appropriate to place of work

▪ Demonstrates an understanding of the significance of and obligations under Te Tiriti o Waitangi, including how to apply Te Tiriti principles in a meaningful way in the nursing role

▪ A commitment to achieving equitable outcomes for Māori

▪ A personal commitment to on-going learning and development including attainment/maintenance of PDRP

▪ Comprehensive experience in both inpatient and community mental health settings

▪ Full driver’s license
