Registered Nurse - Advancing Wellness at Home Initiative
Location: Wellington Regional Hospital (Ewart Building) and Kenepuru Community Hospital, with community-based practice across the Capital, Coast and Hutt Valley district.
Contract: Fixed Term (10 August - 31 October 2026), Full Time (1.0 FTE)

This is an exciting opportunity to join the Advancing Wellness at Home Initiative (AWHI), an innovative Early Supported Discharge service helping people transition safely from hospital back into their homes and communities. As a Registered Nurse, you will work alongside an experienced inter-professional team to provide comprehensive assessment, clinical intervention, care coordination, health education and wellness-focused support for people recovering from illness, injury, stroke, frailty and other complex health conditions.
No two days are the same. You will work across hospital and community settings, supporting people and whānau to achieve their goals, maintain independence, and improve wellbeing. You will build strong relationships with patients, whānau, primary care providers and community agencies while contributing to a service that keeps people well and thriving at home wherever possible
About the Team/Service Location
AWHI is part of the Older Adults, Rehabilitation and Allied Health (ORA) Service and provides Early Supported Discharge across medical, rehabilitation and stroke pathways. Our inter-professional team includes Registered Nurses, Physiotherapists, Occupational Therapists, Speech Language Therapists, Social Workers, Dietitians, Psychologists and Allied Health Assistants who work collaboratively to support people across Wellington, Porirua and the Kāpiti Coast
About you
You will be a motivated and adaptable Registered Nurse who enjoys working autonomously while contributing to a highly collaborative team environment.
- Registered with the Nursing Council of New Zealand and hold a current Annual Practising Certificate (APC).
- Minimum of three years' clinical experience as a Registered Nurse, with a proficient or expert PDRP level.
- Demonstrated commitment to Te Tiriti o Waitangi and achieving equitable health outcomes for Māori.
- Experience working with older adults and people with complex health needs in community or rehabilitation settings.
- Full New Zealand driver's licence and confidence working across multiple locations and community environments.
