About the Role 

If you are passionate about making a difference in disabled people’s lives, then this job is an absolute must for you. An incredibly supportive/ experienced and knowledgeable team with an experienced Service Leader will surround you as you start and grow into this challenging role of completing needs assessments, identifying support required, and then co-ordinating appropriate services to meet the identified disability related needs. 

Liaising with providers and contracting them to support the person and their Whanau after developing relationships and understanding with our client/ Whanau. This is all about partnership and working within the opportunities available. From Individualised Funding, to relief supports, residential services, in home supports, behaviour support, and so much more is what Capital Support can offer to our eligible community. 

This job is not for the faint hearted. You will learn at a fast pace and learn skills that are not just beneficial to your work, but your Wairua (spirit), your Tinana (Physical wellbeing), Hinengaro (Mental wellbeing), and Whanau (Family/ community).

About the Team/Service/ Location 

Capital Support is an under 65 y/o needs assessment service covering the greater Wellington region funded by Ministry of Social Development contracting Health NZ in this particular region. This service works in partnership with disabled people to achieve a better future for people with disabilities. 

We have a position available in the team based at Kenepuru Hospital but reaching out to the broader community served by Capital and Coast Te Whatu Ora. 

A team of 17 including 1.8 FTE Admin, 1 casual administrator, and 12.75 FTE Service Facilitators, and a Service Leader… A supportive team with a diverse range of skills/ qualifications and experience. Team culture has been a big focus over the past 12 years, the team is a very strong, close, respectful and collaborative team.

About you 

Experience within the disability sector and/or of needs assessment and service coordination would be an advantage.  The work will involve negotiating and facilitating complex service requirements. You will also have outstanding communication and relationship-building skills, empathy, computer literacy and dedication to make a difference to people's lives.

This role will suit a person experienced in problem solving, inspiring a culture of quality, whilst enjoying working in a team environment as well as working autonomously. You’ll be an achiever who can see what needs to be done and gets on with doing it. You’ll be committed to excellent outcomes for clients through working as part of a team.

Capital Support will provide ongoing training and support with professional development opportunities.

Required: Current Driver’s License

A relevant qualification that could be applied to Disability Support Services is preferred but not compulsory.
