Kōrero mō te tūranga – About the role
Advancing Wellness at Home Initiative (AWHI) are looking for a Team Coordinator to support the Team Leader to continue to expand and develop the service. The Coordinator role is based at Kenepuru Hospital and supports across all AWHI Streams.
The role involves overseeing and supporting the prioritisation of clinical workloads on a daily basis as well as a variety of non-clinical tasks such as supporting with rosters, performance reviews, recruitment and service development. There will also be an expectation to support with clinical workload when needed. Ideally, this role would be combined with a clinical role within the team. This clinical aspect of this role is based in the community with an in-reach component, working in the hospital to identify patients who will benefit from the service.
Mō tō mātou rōpū – About our team
AWHI aim to facilitate quicker discharges from hospital and to provide Nursing and Allied Health input/rehabilitation services to patients in their own home. The team work predominately with older adults who have been admitted to hospital with a medical condition, following a stroke, or as a result of an injury to help them achieve their rehabilitation goals.
You will be supporting to lead a friendly, fun and enthusiastic team from a mix of professional groups including Physiotherapists, Occupational Therapists, Speech Language Therapists, Social Workers, Nurses and Assistants. We offer on-going professional development opportunities including profession specific training and inter-professional practice support. You will also have access to professional support through the relevant professional leaders as well as clinical supervision. Cultural professional supervision is available from Māori and Pacific Health teams.
Moū – About you
We are looking for an Allied Health Clinician or Registered Nurse who has current registration/APC and has recent clinical experience working with people older adults, people who have experience strokes or in rehabilitation/community settings. The ideal candidate will have a minimum of 5 years’ experience and will hold a current NZ driver’s license. You will be organised, have excellent communication skills and enjoy supporting the development of others. Previous leadership or service improvement activity experience would be an advantage as would an understanding of ACC.
