Team Leader - Advancing Wellness at Home Initiative

Contract: Fixed Term Secondment covering Parental Leave (December 2025 – October 2026), Full Time: Monday to Friday with On Call

About the Role 

Please note that this role is only available for current Health New Zealand / Te Whatu Ora staff.  Any non-Te Whatu Ora applications will be declined. 

This role provides roster oversight, has delegated financial responsibility for this cost centre and ensures the individual clinicians receive the appropriate support and guidance. Maintains and report on KPIs including ACC revenue, incident reporting. Acts as the primary contact for all stakeholders including clinical leads across CAHOA and the wider health service both primary, secondary and tertiary levels.

About the Team/Service Location 

Advancing Wellness at Home Initiative (AWHI) is the Early Supported Discharge (ESD) Service for Capital, Coast & Hutt Valley District of Health New Zealand – Te Whatu Ora. Healthcare in New Zealand is funded through two main streams – Ministry of Health (MOH) funding which includes care for any illness or long term/chronic conditions and Accident Compensation Corporation (ACC) funding which covers any accident or injury. AWHI provides services funded by both MOH & ACC. 

AWHI are an interdisciplinary team that are made up of Physiotherapists, Occupational Therapists, Social Workers, Speech Language Therapists, Registered Nurses and Allied Health Assistants. The team are split across two sites with AWHI North covering Kenepuru Hospital & AWHI South covering Wellington Hospital. Once patients are discharged they are picked up by North or South Team depending on their location. 

About you 

Must have clear leadership and people management skills, possess both clear verbal and written communication.  Has a pragmatic, problem solving focus and is able to plan and organize tasks, allocation of resources and meet deadlines.
